News Note 6/2/03 The Colorectal Cancer Network is an excellent organization which is involved in (among other things) legislative efforts on behalf of colon cancer patients, promoting aggressive awareness, screening, and early detection programs, and support groups . They are sponsoring a Survivor's Conference September 12-14, 2003 in Baltimore, MD. I expect it to be quite good, an opportunity to attend informative presentations by oncologists and other medical professionals, as well as meeting other colon cancer survivors/caregivers from across the country.

    Introduction
After being diagnosed with colon cancer in March, 2001, I began looking for information on the internet about the subject. I was able to quickly locate lots of very good websites with plenty of material about colon cancer diagnosis, prevention, prognosis, chemotherapy, etc., but experienced considerable difficulty in finding a single site which summarized experimental therapies under development specifically oriented to colon cancer in a way that I found useful. I soon discovered that unfortunately my doctors were not always aware of the experimental drugs and trials I was finding, and concluded that if I didn't find out more about these things myself, no one else was going to do it for me. Feeling overwhelmed with unorganized information scattered everywhere, I decided to create for myself an HTML document containing links to the many websites of interest in order to help me organize my own thoughts and research. It occurred to me that this document might be of use to others as well, so I decided to put it up on the Web. You won't find any fancy graphics or other clever programming on this page, as I have little time to create such luxuries. Some sections of the page are better developed than others, reflecting my interests and time to work on them. What I would consider the "best developed" sections would be those on Biologic Agents (Monoclonal Antibodies, Cancer Vaccines) and the sections on Celebrex/Lovastatin (Celebrex is one of the few "alternative therapies" I could find which have serious suggestions of efficacy in the peer reviewed medical literature, although to date there have been no clinical trials proving efficacy in humans for colorectal cancer). If you are interested in "what is going on" with new drug development, but are having a hard time in "putting it all together", you have come to the right place. I plan on updating the page regularly, you can always see the latest date this has occurred at the top. The date you will see next to each item on the page is the date I first located that item - as of that date, the link was working.

If you would like to e-mail me, my address is altman23@cs.com. I plan on improving the website as I have time, reorganizing and creating seperate pages for some of the topics. Any suggestions for additions to the page or comments are always welcome.

    Getting Started

Here are some general references to help you get started with your search for information. One of the most important sites is the National Cancer Institute Homepage - see their booklet Cancer of the Colon and Rectum . After you've finished that, you will be ready for the more detailed Diagnosis and Staging Page .

  12. 4/9/01 Colon Cancer Treatment PDQ, from NCI This is a much more technical summary of colon cancer from the NCI and might be considered a starting point for the medical literature. If you are just learning about colon cancer, you may want to postpone this one for later. It gives a basic summary of colon cancer: stages, treatment options, clinical trial results, literature summary. It contains some of the nasty "survival" stats, so if you're uncomfortable with that please do not click here. Keep in mind the data they calculate these things from is at least 5 years old and does not take into account the possibility some of the new therapies mentioned in this site may significantly improve survival.

As I mentioned, I had drug resistance tests done, which is why I chose not to do chemo/Adria right off the bat, although the honor could have been mine. Most medical journals do not have free access to online articles without being a subscriber - some do, however, and I've included both types here. You can usually view the abstracts each month for the journals which do NOT have access to full articles to at least get an idea of what is going on for the month.

Therapy for Liver Metastasis Go to Page

Most patients with late stage colon cancer at some time or another will have liver metastases. There are several ways of dealing with these: Chemotherapy, Surgery, RF Ablation, and Cryotherapy. It would appear that RF ablation is overtaking Cryotherapy as the main "nonsurgical" method of liver met removal.

  257. 9/13/01 Pulmonary metastasectomy: might the type of resection affect survival? Mineo TC, Ambrogi V, Tonini G, Nofroni I. J Surg Oncol 2001 Jan;76(1):47-52 CONCLUSIONS: The type of resection did not disclose statistically significant differences on survival. Minimal surgery, especially by laser device, is recommended for less morbidity.

  258. 7/7/01 Radiosurgery device shows promise in treating nonresectable lung cancers A radiosurgery instrument, known as the CyberKnife, may offer an additional treatment option for patients with nonresectable forms of lung cancer. Dr. Crownover said that "anyone who has non-small cell lung cancer or metastatic disease is potentially eligible for the study." However, "the tumor has to be less than 5 cm and the patient must not have had prior radiation to the area," he noted. In addition, "if the patient appears to be a surgical candidate then that is the route they usually go," he said.

  259. 8/30/01 Cryotherapy might be first-line treatment for early lung cancer Thursday, Aug 9 2001 Cryotherapy performed through a rigid bronchoscope is both safe and effective in the treatment of patients with early lung cancer, French researchers have found. Dr. Jean-Michel Vergnon, of Hopital Nord in Saint-Etienne, France, and a multicenter team tested the safety and long-term efficacy of cryotherapy in 35 patients with histologically confirmed early superficial bronchogenic carcinoma, seven of whom had lesions at multiple sites. A complete histologic response was noted in 32 patients (91%) at 1 month and in the same number at 1 year, with "no severe adverse effects," Dr. Vergnon's team reports in the July issue of Chest. In seven of these patients, local recurrence was observed within 4 years. Of 22 patients for whom followup data were available at 48 to 89 months, 11 were still alive.

    Company Websites

As noted earlier, if you want to find out about clinical trials for a drug, you should identify the company manufacturing the drug, locate the company website and contact information, and give them a call. Here, I have collected links for a number of companies with CC drugs in development. I have found if you are trying to follow a particular drug's development process, the "stock" BB's are often the best source of information - they have all sorts of rumors and info floating around sometimes weeks before it is officially released. So for each company I have included links to the Yahoo message board, and the Raging Bull message board. I have also included a link to the "PR Newswire" daily news.

    Biologic Agents Go to Page

Biologic therapies are one of the hottest areas of Cancer research, because unlike the traditional chemotherapies, they have few side effects. There are 2 main types of biologic agents, monoclonal antibodies and cancer vaccines. Here, I have tried to locate the main drugs/companies and give a few articles from the popular press which caught my attention. Along with the links given in the "Company Websites" section, I have also included links to the "biospace news" on each compound. This is probably the best developed section of my website, as I have focused most of my search on Biologics.

    AntiAngiogenesis Go to Page
Antiangiogenesis drugs are supposed to work by not allowing the tumor to create new blood vessels, thereby cutting off its supply of blood and killing it. This section contains the references I found, but is not as well developed as the "biologic agents" section because I have been more interested in the antibodies and vaccines lately. I plan to revise this section in the future if I have time.

Serial CEA determinations are appropriate if the patient is a candidate for aggressive surgical resection, should recurrence be detected. Abdominal and pelvic CT scans should be utilized only when there are clinical indications of possible recurrence. Patients whose disease progresses during 5-FU-based therapy should be treated with irinotecan or encouraged to participate in a phase I or phase II clinical trial.
