7 Easy Steps to Starting A Drug Screening Program:

1. Call 1-508-634-7748 and ask for a
   Landata Research, Inc., Company/Consumer Agreement.
2. Send a $150 check made out to Landata Research, Inc., for the set-up fee to:

3. Select which drug screen panel (5-7-9) you wish to implement.
4. Landata will provide you a list of convenient collection sites.
5. You will receive, via mail, Chain of Custody forms.
6. You will then be prepared to implement drug-screening procedures.
7. Landata Research, Inc., will bill you for services performed.
