Filling Out the Club Visit Form

When the Area governor makes an official club visit (which is required at least twice annually) there is a form to be filled out for submission to District and International. The form is intended to help the visiting Area Governor notice thigs at the club.

Unfortunately the form is in rather small print inorder to fit on a single sheet of paper. The form can also be rather intimidating. It asks a number of questions that most clubs do not have handy. With proper pre-planning all the questions are fairly easy to answer. Area governors are also supplied with a book on how to fill it out and the significance of each item. Unfortunately, I can't find additional copies to hand out.

Therefore, I am entering and summarizing the material here for convenient access.
