$25 fee for no-shows and other absences (unless appointment canceled
at least 24 hours in advance).
Cash (exact amount/no change made) or money orders accepted
and due at time of session (or before, depending on referral source).
SUBSTANCE ABUSE EVALUATION ($75)
- Comprehensive assessment and screenings by a Licensed Addiction Counselor with training in differential assessment, use of the Diagnostic and Statistical Manual and mental status exams. Findings and recommendations provided to the client or the referral source with a signed release of information form the same week as the appointment.
